I guarantee these poor ratings are from people who just agreed to getting the insurance because it was more affordable than the ACA and completely ignored the agent when they explained limitations and excluded covered items. It is also not ObamaCare insurance so it won't cover you for mental health treatments, maternity, and rehab. This is private insurance which means it has waiting periods for pre-existing conditions.
The standard benefit (even if they went out of network for the x-ray) is $75. These claims that they only paid $27 towards an x-ray is the furthest thing from the truth.
